How do I qualify?
In order to qualify for this service, you must have an ACCESS card which is issued by the Pennsylvania Department of Public Welfare.
If you have an ACCESS card,
complete Section I of the Medical Assistance Transportation Program
Application and have your physician complete and sign Section II.
